To make it easier for you to apply we've detailed below the steps you need to follow along with the forms you need to complete and a note of any other documentation you need to provide the Prudential.
Just to let you know, the Prudential cannot set up your annuity until they have received all the money from your pension scheme(s) plus all the forms and other relevant items. You should know that moving your pension fund from one company to another can take several weeks but we will keep you updated on the progress of the fund transfer throughout your annuity application.
Please send the following items to your current pension scheme:
Covering letter from you to your pension scheme
- Please write to your pension provider to explain that you have purchased an annuity from Prudential and to pass on the enclosed forms (explained in section 2 and 3) which they must complete.
- On your letter to them, it would be helpful to quote your scheme membership number (if you know it) and your date of birth.
- If you are using money from more than one pension scheme to buy your annuity, please write to all the pension schemes with copies of each of the forms.
- Each pension company may ask for different documents.
Immediate Benefits Personal Pension Transfer-in Authorisation
This form collects vital details about your pension fund and the scheme to which it belongs.
- Do not complete this form yourself.
- Please send it to the Trustees, or administrators of your pension scheme with the covering letter explained in section 1. Ask them to complete this form and return it to the Prudential when they send your pension fund payment cheque. Telegraphic transfer details are also shown on this form.
Payment form
This form will help the Prudential to match your pension fund payment to your application.
- Do not complete this form yourself.
- Please send it to the Trustees, or administrators of your pension scheme with your covering letter (explained in section 1) and the Immediate Benefits form mentioned above. Ask them to complete this Payment form and return it to us when they send your pension fund payment cheque. Telegraphic transfer details are also shown on this form.
Please send the following items to the Prudential:
Please send the Prudential the following original certificates or legal documents.
- Annuity application and payment instruction.
- A copy of your quotation.
- Your birth certificate or a completed Verification Form (You can get a Verification Form from your Financial Adviser or your Solicitor).
- Birth and marriage certificates, or civil partnership document for your named dependant or suitable Verification Form.
- Marriage certificate, or civil partnership document and/or decrees absolute, or dissolution order and/or deed polls. These are only needed as proof of name change if you and/or your dependant now use a different name to the one given on your birth certificate or pension scheme documents.
If you send the Prudential any personal documents they will return them to you, or us by recorded delivery as soon as possible (usually within 3 days).
Cancellation notice and form – Please keep this in a safe place. Only sign and return the form if you want to cancel your annuity application within 30 days of receiving the first quote.
